Textbook Change Request

Course Coordinators (CC):

Please complete section A for courses under your jurisdiction.  For a listing of what courses that includes, please refer to EPPS-06 (pages 5-8).


Instructors of Record (IoR):

If you are completing this form, it means you wish to change the textbook for an upcoming course you will be teaching. To get started, please start at section B.

Keep in mind, change requests MUST be submitted at least 1 month prior to the textbook request deadlines listed below or they will be automatically denied.

 

A list of currently approved textbooks in use can be found here.

 

Textbook request deadlines are as follows:

Spring Requests are due October 1
Summer Requests are due February 15
Fall Requests are due March 1

Course Coordinator:
If you are changing a textbook, please make sure to submit all required information (see below for example).

Instructor of Record:
If you are requesting a textbook be changed, please make sure to submit all required information (see below for example). Once your request is received, it will be forwarded to the appropriate CC. Once a decision is made, you will be notified via email.  Remember, you MUST submit your change request at least 1 month prior to the textbook request deadlines (found above).  Failure to do so will result in an automatic denial.
